Teen & Adolescent Forms (ages 10-17)
Please click on all of the forms below to download them. Once you have downloaded them, please follow the directions below about how to complete and return the forms to us. Additionally, please send us all previous documents you have – Such as: Any prior evaluation reports, Reports from doctors or other therapists, Educational Reports, etc.
There are three ways to complete and submit these forms:
1. ONLINE: To complete the forms online, you must open each form with a PDF-filler app/program (ex: Adobe; or free online programs such as: PDFfiller.com; PDFSimpli.com, etc.). Please use a PDF-filler ONLY, not an “editor”, because you are only filling it out, not “editing” the PDF. Once you have completed the form, SAVE the files on your device. Repeat this process with ALL of the forms. (A) Email: Please email all of the completed files, along with any additional documents, to: info@horizonspeech.com (B) Upload: Please upload all of the files directly into the form below
2. PRINT & SCAN: To complete the forms, please print them out and complete the information with blue or black ink. Then, SCAN the completed forms (photos of forms will not be accepted) as well as any additional documents (ex: reports, etc.), and email the forms to. Our email is: info@horizonspeech.com. NOTE: If you do not have a scanner, you can go to any office supply type store (ex: Staples, Office Max/Depot, FedEx Office, UPS stores, etc.), as most of them have scanning services. Please call or email us to confirm that we have received your completed forms.
3. PRINT & MAIL: To complete the forms, please print them out and complete the information with blue or black ink. Then, when all of the forms are completed, please place all of the forms in a large manilla envelope and MAIL the forms to us. Please also include any additional documents you may have (ex: evaluation reports, notes from doctors, etc.). Our address is: 780 Cedar Lane, Suite 3, Teaneck, NJ 07666. NOTE: No appointments will be scheduled until ALL of your paperwork has been received by this office. Please call or text us to confirm that we have received your completed forms.
