Adult Forms (Age 18+)
Please click on all of the forms below to download them. Once you have downloaded them, please follow the directions below about how to complete and return the forms to us. Additionally, please send us all previous documents you have – Ex: Prior evaluation reports, Reports from doctors or other therapists, Educational Reports (if applicable), etc.
There are three ways to complete and submit these files:
1. ONLINE: Once all of the forms have been downloaded, please open each form with a PDF app/program (ex: Adobe, or free online apps/programs such as PDFiller.com, etc.). Please make sure that you are using a PDF “filler” ONLY, and not an “editor”, because you are not allowed to “edit” the forms). Then, complete all of the requested information on your screen. When done, please save the file to your device. To submit your completed forms: (A) EMAIL: Please email your completed forms, as well as any additional documents you may have, to: info@horizonspeech.com (B) UPLOAD: Please upload your completed forms (files), as well as any additional documents you may have, directly into the form below
2. PRINT & SCAN: Print the forms, and complete all of the information using blue or black ink. When you’re done, please SCAN the papers (photos of forms will not be accepted, and you will have to re-send them). Then, and email the scanned forms (along with any additional documents you have) to: info@horizonspeech.com Or, upload the files directly into the form below.
3. PRINT & MAIL: Print the forms, and complete al of the information using blue or black ink. When you are done completing the form, place the papers into a large, manilla envelope and mail the forms, along with any additional documents you may have (ex: evaluation reports, doctor’s notes, etc.), and mail them to: Horizon Speech Therapy Services, LLC, 780 Cedar Lane, Suite 3, Teaneck, NJ 07666. NOTE: No appointments will be scheduled until all of your paperwork has been received by our office. Please call or text our office to confirm that we have received your paperwork.
